What is Dermatitis herpetiformis?
- Dermatitis herpetiformis (DH) is a chronic autoimmune skin condition that is characterized by itchy bumps and burning blisters. Those people who develop sensitivity to gluten, like people that are suffering from celiac disease, might get this skin condition. In dermatitis herpetiformis lesions look similar to herpes, but it is a different condition. Taking gluten-free diet may help to manage the condition.
- Dermatitis herpetiformis is sometimes known as the skin manifestation of celiac disease. Dermatitis herpetiformis affects 10% to 25% of celiac disease patients.
- In the word Dermatitis Herpetiformis “dermatitis,” means “inflammation of skin ” and the word “herpetiformis” is used because the lesions in this condition looks like herpes lesions. However, this skin condition is not caused by the herpes virus.

- Dermatitis herpetiformis characterized by cluster of itchy bumps that may be confused with acne or eczema. Blisters can also noticed that can be misdiagnosed with herpes.
- Dermatitis herpetiformis lesions commonly seen on the:
- Elbows
- Hairline
- Knees
- Buttocks
Who does dermatitis herpetiformis affect? Who is at risk?
- Dermatitis herpetiformis can be seen in every age group. However, it is most prevalence occurs between the ages of 30 and 40. Men commonly get it more than women.
- People at slightly higher risk – Having celiac disease, autoimmune diseases that include pernicious anemia, thyroid disease, vitiligo, type 1 diabetes, alopecia areata and Addison’s disease.
What causes dermatitis herpetiformis?
- Dermatitis herpetiformis caused due to an autoimmune reaction when people eat gluten.
- Genetics also play a role in causing dermatitis herpetiformis. The genes HLA-DQ2 and HLA-DQ8 are closely associated with dermatitis herpetiformis.
Signs and symptoms of dermatitis herpetiformis
In dermatitis herpetiformis, people may suffer from one or more of three problems that include: skin problems, problems in your mouth and gastrointestinal problems.
- Skin problems: Skin problems are the most common in dermatitis herpetiformis. Skin problems include red bumps (lesions) and itchy, sometimes burning blisters. Skin lesions are noticed on elbows, buttocks, knees and scalp.
- Oral problems: Dermatitis herpetiformis can be reason for tooth enamel problems. It may lead to discoloration of tooth, horizontal grooves and (rarely) canker sores.
- Gastrointestinal problems: Due to its relation with celiac disease gastrointestinal issues like bloating, cramping, diarrhea, constipation and pain can be noticed among people.
Diagnosis and Tests
Doctor can diagnose the condition by taking medical history and by physically examining your bumps and blisters to know the skin condition. Then go for skin biopsy and/or blood test to see if you have dermatitis herpetiformis. These investigations are helpful to confirm the diagnose and rule out other types of dermatitis.
- Skin biopsy. In this skin sample is taken to check for evidence of dermatitis herpetiformis.
- Blood test. Two antibodies that are commonly found in people with celiac disease include anti-endomysial and anti-tissue transglutaminase. These antibodies are diagnosed with the help of blood test.
Management and treatment
- Dermatitis herpetiformis can be managed with a gluten-free diet and an oral antibiotic called dapsone. Dapsone will help to relieve your itching and typically before 48 hours.
- You have to continue taking dapsone for atleast one to two years to prevent severity of dermatitis herpetiformis and to reduce bumps and blisters from forming.

- Reduce the risk for developing autoimmune diseases.
- Reduce the need for medication to control or reduce the skin lesion.
- Reduce the risk of intestinal lymphoma.

Complications/side effects for dermatitis herpetiformis treatment
- You’ll need to go for regular blood tests if you are taking dapsone. If the skin lesions of dermatitis herpetiformis continue despite the gluten-free diet and medication, then you need to remove iodine from your diet as well.
