LIVER ABSCESS AND ITS AYURVEDIC MANAGEMENT

INTRODUCTION:
It is defined as a pus filled mass in the liver that can develop from injury to the liver or an intrabdominal infection disseminated from the portal circulation. It is caused due to formation of germs like bacteria, parasites and fungus. Liver abscess can cause serious problem if left untreated. Infection can spread TO the liver through biliary tree, hepatic vein, portal vein or as a result of trauma.
CAUSES OF LIVER ABSCESS:
- Abdominal infection such as appendicitis, perforated bowel and diverticulitis.
- Infection in the blood
- Injury to the liver.
- Bacterial causes of liver abscess are streptococcus, Escherichia, Klebsiella species.
- Infection in the bile duct.
- Endoscopy of the bile draining tubes.
TYPES OF LIVER ABSCESS:
- Pyogenic liver abscess: Pyogenic means pus.It is a pus filled pocket of fluid within the liver.
- Amoebic liver abscess: It is the collection of pus in the liver in response to an intestinal parasite caused by Entamoeba histolytica.
- Fungal abscess: It is due to the candida albicans and occur in person who have prolonged exposure of congenital and acquired immunodeficiency
- Iatrogenic abscess: It is a complication of hepatic artery ligation for tumor.

In Ayurveda it is compared with Yakrit Vidradi. The dosha located around the organ or bone tissue it gets vitiated and affects the twak, rakta, mamsa and meda and will produce swelling and gradually develops into bigger size and pus is formed inside the cavity. Vidradhi are deep located and associated with pain.

INTRODUCTION
Lichen planus is a skin condition which is characterized by inflammation, swelling and irritation of the skin, hair, nails and mucous membranes of the body. It is triggered by the immune response of the body. However, there are other reasons also such as allergens, stress, genetics, etc. The skin appears purplish red with flat bumps over it. There occurs itching and irritation and the patches develop over weeks. The condition occurs on the mucous membranes of mouth and vagina. Lichen planus doesn’t spread from one person to another. When the condition is mild, it can be managed by self-care measures. However, there are certain oral and topical medications which help in management of lichen planus.
WHAT ARE THE CAUSES OF LICHEN PLANUS?
The exact reason for the occurrence of lichen planus is not known but it is believed to be due to an abnormality of the immune system. Normally, the immune system helps in protection of the body against invasion of the foreign micro-organisms. But in lichen planus, the cells of immune system start attacking the body’s own cells and causes damage to the body. The various factors by which lichen planus is caused are:
- Infection of hepatitis C
- Having a genetic predisposition to lichen planus
- Flu vaccine
- Herpes zoster
- Certain medications such as non-steroidal anti-inflammatory drugs (NSAIDS), beta-blockers, arthritis medicines
- Exposure to certain chemicals, metals and pigments
- Emotional or physical stress
WHAT ARE THE SIGNS AND SYMPTOMS OF LICHEN PLANUS?
The common signs and symptoms of lichen planus are:
- Intense itching
- Presence of purplish red bumps on skin
- Painful sores in mouth and vagina
- Skin rashes present on forearm, ankle, genitals and wrist
- Blisters breakdown to form crusts or scabs
- Loss of hair
- Nail damage
- Loss of nails
- Lacy white patches in mouth, tongue or lips
- Dryness in mouth
WHAT ARE THE COMPLICATIONS WHICH ARISE WITH LICHEN PLANUS?
Lichen planus is associated with some of the following complications:
- Discoloration of the affected skin
- Scarring of vulva or vagina
- Severe pain during sexual intercourse
- Other skin infections
- Increases the risk of squamous cell carcinoma
- Presence of sores in mouth
- Oral cancer
- Sexual dysfunction
- Loss of hearing if lichen planus occurs in ear canal
HOW THE DIAGNOSIS OF LICHEN PLANUS IS MADE?
The doctor will ask about the signs and symptoms of the disease. He will do a physical examination of the patient and ask about the family history. He will make the patient do some of the tests to evaluate the disease. The tests recommended are:

DEFINITION:
Stomatitis is defined as the inflammation of mouth and lips. It affects the mucous membrane of mouth and lips, with or without oral ulceration.
CAUSES OF STOMATITIS:
- Dry mouth
- Trauma from ill-fitting dentures or braces
- Tobacco use
- Infection such as herpes simplex viruses or herpes zoster.
- Side effects of chemotherapy, radiation
- Weakened immune system.
- Deficiency of Vitamin -B12, zinc, Iron.
- Biting of cheeks

INTRODUCTION
Otosclerosis is a condition which is characterized by loss of hearing. Oto means ear and sclerosis means abnormal hardening of bone tissue. In this, the bones of middle ear get ossified due to remodeling or their abnormal growth. There are three small bones present in middle ear – malleus, incus and stapes. The irregular bone remodeling leads to growth of ear bone in unwanted areas. This bone remodeling process occurs normally in body to manage wear and tear of the bone. Hearing loss, ringing in ears and dizziness are some of the most common symptoms of otosclerosis. The hearing loss can be mild to severe. The treatment of otosclerosis includes use of hearing aids and surgery.
WHAT ARE THE CAUSES OF OTOSCLEROSIS?
An ear consists of three parts – external ear, middle ear and inner ear. The external ear receives all the sound waves and transmits it to the middle ear. A middle ear contains three bones such as malleus, incus and stapes. These three bones are connected to each other and help in amplification of sound waves. These amplified sound waves are then transmitted towards inner ear. Inner ear is also known as cochlea. In otosclerosis, the bone remodeling leads to fusion of the three bones with each other and the surrounding bony tissue. This leads to non-transmission of sound waves and thus causes loss of hearing.
WHAT ARE THE RISK FACTORS OF OTOSCLEROSIS?
The exact reason of the occurence of otosclerosis is not known but some factors tend to increase the risk of a person developing otosclerosis. Following are the risk factors of otosclerosis:
- Between the age group of 10 to 45 years
- Having a genetic predisposition towards otosclerosis
- Women are at more risk of developing otosclerosis in comparison to men

HOW THE DIAGNOSIS OF OTOSCLEROSIS CAN BE DONE?
One should go to an ENT specialist for having a difficulty in hearing. The doctor will look closely at the ear and ask about the signs and symptoms. He will ask about the patient’s family history. He will do the patient’s hearing test for knowing the intensity of hearing loss. The tests include an audiogram and a tympanogram. In some cases, X-ray and CT scan of the ear are also done.

Otosclerosis comes under the category of Karnaroga. There are three doshas which are present in body – vata, pitta and kapha. Lump of fatty tissue which grows under skin is known as Lipoma. It is not hard and feel rubbery when touched and tends to move with slight pressure of fingers. It is usually painless and can appear anywhere on body but most commonly appears on trunk, back, shoulders, neck and arms. They tends to grow slowly and are non-cancerous. Most of the lipomas do not require any treatment but if individual feel bothered it can be removed.
WHAT ARE THE CAUSES OF LIPOMA?
Proper cause of lipoma is not determined but some conditions can contribute in formation of lipoma in the body. The conditions includes:
- Gardner Syndrome : It is a form of Familial adenomatous polyposis (FAP) which causes lipomas along with range of health problems.
- Dercum’s Disease : It is rare disorder that causes growth of painful lipomas mostly on legs, arms and trunk. It is also known as Adiposis dolorosa or Anders’ syndrome.
- Madelung’s Disease : This condition is found mostly in men with drinking problem. It is also known as Multiple symmetric lipomatosis which causes growth of lipomas around the shoulders and neck.
- Hereditary Multiple Lipomatosis : It is also known as Familial multiple lipomatosis and is an inherited condition.
WHAT ARE THE SYMPTOMS OF LIPOMA ?
They are generally painless but can be uncomfortable if press against nerve or develop near joint. The lipoma are usually :

- Myelolipoma : These types of lipomas contains fat and tissues that produces blood cells.
- Pleomorphic : These type of lipomas contain fat cells of different shapes and size.

OVERVIEW
Peptic ulcers are the open sores or ulcers which are present in the inner lining of the stomach and first part of small intestine. They mostly occur due to bacterial infection of the digestive system. The infection is caused by the bacterium Helicobacter pylori or long term use of non-steroidal anti-inflammatory drugs like naproxen, ibuprofen, etc. Peptic ulcers are of two types – gastric and duodenal ulcers. Gastric ulcers are present in stomach and duodenal ulcers are present in the first part of small intestine. Pain in stomach is one of the most common symptoms of peptic ulcers. Intake of spicy foods and excessive stress don’t cause peptic ulcers but makes the condition worse. Peptic ulcers can be managed by certain medications. However, good eating habits and healthy lifestyle play a very important role in its management.
WHAT ARE THE CAUSES OF PEPTIC ULCERS?
Normally, in digestive system, the stomach wall produces acid called hydrochloric acid. This acid helps in digestion of food and protects the body against foreign micro-organisms. For protection against this acid, the stomach wall produces mucus. This mucus covers the stomach lining and prevents it from acid erosion.
